FORMS THAT YOU MAY NEED DURING THE SEASON:
Injury Incident Report To be completed by Certified Athletic Director, Coach or Site Official and should be filed within 24 hours to the CYO office.
General Incident Report To be completed by Certified Athletic Director, Coach
or Site Official for violations of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO office.
FORM FOR KIDS WHO DO NOT ATTEND REQUESTING CATHOLIC SCHOOL OR PARISH:
Eligibility & Exception Form (for kids who do not attend a Catholic school or parish, or for kids that want to play for a different club. Kids are NOT allowed to practice or play with the team UNTIL they have been notified that the exception was approved by the Rules and Compliance Committee.)
