FORM FOR KIDS WHO DO NOT ATTEND THE REQUESTING CATHOLIC SCHOOL OR PARISH:
Eligibility & Exception Form for Participants (if you do not attend a Catholic School or Parish you must fill out this form. A child is NOTallowed to practice or participate in any matter with the team UNTIL you have been notified that the exception was approved by the Rules & Compliance Committee.)
FORMS THAT YOU MAY NEED DURING THE SEASON:
Injury Incident Report To be completed by Certified Athletic Director, Coach or Site Official and should be filed within 24 hours to the CYO office.
General Incident Report To be completed by Certified Athletic Director, Coach
or Site Official for violations of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO office.
