Swim Coaches Manual (Complete) Revised for 09-10
Eligibility & Exception Form
For kids who do not attend a Catholic school or parish.
Kids are NOT allowed to practice or play with the team UNTIL
they have been notified that their Exception was
approved by the Rules and Compliance Committee.
Injury Incident Report
To be complete by a Certified Athletic Director, Coach or Site Official
and should be filed within 24 hours to the CYO Office.
General Incident Report
To be completed by a Certified Athletic Director, Coach or Site Official for violations
of the "CYO Code of Conduct" and should be filed within 24 hours to the CYO Office.
