Eligibility & Exception Form (for kids who do not attend a Catholic school or parish, or for kids that want to play for a different club. Kids are NOT allowed to play or practice with a team UNTIL they are notified that their exception was approved by the Rules and Compliance Committee.)

Health Form (must be filled out and signed by the doctor every year).
