REGISTRATION FORMS

STEPS TO REGISTER BY MAIL OR FAX:

Mail registration form with a $50.00 non-refundable fee to our Portland office OR fax the registration form to the Portland office if paying by credit card. (Credit card charges must pay in full at time of registration.)

Final payment is due 30 days after confirmation of session. Payment in full is required for registration received after June 14, 2008. Please note that the full fee must be paid prior to your child's camp attendance.
A $50.00 Nonrefundable registration fee per child must accompany the registration form and is applied towards the total camp fee. (Camp fee covers food, program supplies, round-trip transportation, staff wages and facility expenses.) Camp Howard reserves the right to cancel any program for which there is insufficient registrations.

CIT I & CIT II Applicants

If you want to be a CIT (Counselor in Training) please follow the steps below:

1. Download the application for I or II, complete all sections and
fax or mail to the Portland office as instructed on forms.

2. Download the health form and complete. Must also
be signed by your doctor and submitted to the CYO office before
you can attend camp.

Health Form
The health form must be completed and returned to our Portland office no later than TWO WEEKS PRIOR to your child's camp attendance. You may mail or fax the health form to the Portland office. (FAX # 503-231-9531) ACA standards require that each camper have a completed health form before attending camp.

Confirmation

Upon registration into a Camp Howard session you will be emailed a confirmation packet and any balance due. It will include transportation information, what to bring and other general information about camp.

Refund Policy
The camp deposit is nonrefundable. 100% of the camp fee
(minus the deposit) is refundable if written request is received in the
Portland office at least 2 weeks prior to camp attendance. 50% of
the camp fee (minus the deposit) is refundable if written request
is received in the Portland office 1 week prior to camp attendance.
